Required information

For quick use of this form, please have the following information ready 

Insurance policy certificate.  
This will contain key information like Name and address of the policy holder, Policy number and the Insurer.  

Its also important to note that the Insurer and Broker are two seperate components of a policy.   So, for example, CTH may be your Broker, but Tower insurance are your Insurers as per the Certificate. 

Please also have your vehicle log book to hand.  This will also hold key information which will assist us in identifying the correct glass required for your specific vehicle model.  Including Chassis/VIN number and Registration.  

One final thing to note when filling out this form is the image upload section.  
This is not for proof of damage, but for us to identify the correct glass/windscreen.  So please take a picture from the front of your vehicle of the whole windscreen, and one more of the upper centre section of the windscreen.
